Why Multi-Platform Fragmentation Creates Risk

The Interactions That Matter in a Typical Stack

Blood Pressure Stacking

The most common clinically significant risk in multi-med men isn't a dramatic drug interaction — it's additive blood pressure lowering from multiple mild contributors. GLP-1 medications can reduce blood pressure by 3–5 mmHg. TRT can either raise or lower BP depending on the individual. PDE5 inhibitors lower BP by 5–8 mmHg. Alpha-blockers for BPH lower BP by 5–10 mmHg.

Individually, none of these are concerning. Combined, a man on all four could experience clinically significant hypotension — dizziness on standing, lightheadedness, fatigue — that no single prescriber would predict because each sees only their drug's contribution.

CYP3A4 Metabolism Competition

Sildenafil and vardenafil are metabolized by CYP3A4 in the liver. Several common medications inhibit CYP3A4, raising PDE5 inhibitor blood levels and potentially increasing side effects. Common CYP3A4 inhibitors that multi-med men might encounter: fluvoxamine (an SSRI), ketoconazole (antifungal), certain macrolide antibiotics (clarithromycin, erythromycin), and grapefruit juice.

If your GLP-1 prescriber puts you on an antibiotic for an unrelated infection, and you don't mention your sildenafil prescription from another platform, the antibiotic could raise sildenafil levels significantly.

Hematocrit Elevation

TRT increases red blood cell production — hematocrit values above 54% increase thrombotic risk. Dehydration from GLP-1 side effects concentrates the blood further, artificially raising hematocrit. If your TRT provider monitors hematocrit but doesn't know you've started a GLP-1 (which causes fluid shifts), they might miss that your elevated hematocrit is partly dehydration-mediated.
