It's not universal — some men tolerate TRT with no hair impact — but it's common enough that anyone considering testosterone replacement and who values their hair should understand the mechanism and the mitigation strategies before starting.
Why TRT Affects Hair
The culprit is DHT — dihydrotestosterone. Testosterone is converted to DHT by the enzyme 5-alpha-reductase, primarily in the skin, scalp, and prostate. DHT is approximately 3–10 times more potent than testosterone at the androgen receptor.
