Men's Metabolic Health at Midlife: The Numbers That Actually Matter

Fasting Insulin (The Early Warning)

Most standard blood panels check fasting glucose. But glucose is a late-stage marker — it stays normal for years while your body compensates by producing more and more insulin. Fasting insulin catches the problem earlier. Optimal is below 5 µIU/mL; anything above 10 suggests your body is working overtime to manage blood sugar, even if glucose reads normal.

Triglyceride-to-HDL Ratio

Example: Triglycerides 180 / HDL 35 = ratio of 5.1 — this warrants intervention regardless of what your total cholesterol or LDL numbers look like.

Testosterone (It's a Metabolic Marker Too)

Low testosterone and metabolic syndrome have a bidirectional relationship. Obesity drives testosterone down (via aromatase activity), and low testosterone promotes further fat accumulation and insulin resistance. Men with metabolic syndrome are significantly more likely to have low testosterone, and men with low testosterone are more likely to develop metabolic syndrome.

If your metabolic markers are trending in the wrong direction, check your testosterone. If your testosterone is low, check your metabolic markers. They're part of the same picture.

The Midlife Inflection Point

For most men, the 35–50 age range is where metabolic health either stabilizes or starts declining. The physiological changes are gradual: testosterone drops 1–2% per year, basal metabolic rate declines, lean muscle mass decreases if not actively maintained, and the cumulative effects of diet, stress, and sedentary habits compound.

The men who arrive at 50 metabolically healthy are typically the ones who addressed these trends in their late 30s and 40s — not the ones who waited for a diagnosis.
