Most TRT side effects announce themselves — acne, mood shifts, water retention. Elevated hematocrit doesn't. Your blood gets thicker, your cardiovascular risk increases, and you feel nothing until something goes wrong. This is why hematocrit monitoring is the most safety-critical element of TRT management.
What's Happening Physiologically
Testosterone stimulates erythropoietin (EPO) production in the kidneys, which signals the bone marrow to produce more red blood cells. More red cells means higher hematocrit — the percentage of blood volume occupied by red blood cells. In physiological testosterone ranges, this effect is modest. On TRT, particularly at higher doses, the effect can push hematocrit above safe thresholds.
Normal hematocrit in adult males: 38–50%. On TRT, values of 48–52% are common and generally well-tolerated. Above 54%, the blood's viscosity increases enough to raise the risk of thromboembolic events — blood clots that can cause deep vein thrombosis, pulmonary embolism, or stroke.
Risk Factors That Compound
- Higher TRT doses: Dose-dependent erythrocytosis. Men on 200mg/week see more hematocrit elevation than those on 100mg/week.
- Dehydration: Concentrates the blood, artificially raising hematocrit. GLP-1 medications that cause nausea and reduced fluid intake can contribute.
- Sleep apnea: Chronic intermittent hypoxia from untreated sleep apnea independently elevates EPO and hematocrit. Combined with TRT, the risk multiplies.
- Altitude: Living above 5,000 feet increases baseline hematocrit. TRT pushes already-elevated values further.
- Smoking: Carbon monoxide from smoking raises carboxyhemoglobin, which triggers compensatory red blood cell production.
- Age: Older men have less bone marrow reserve but may still produce excessive red blood cells on TRT.
The Management Protocol
Hematocrit Management Ladder
- Below 50%: Normal range. Continue current protocol and monitor at regular intervals.
- 50–52%: Watch zone. Ensure adequate hydration (64+ oz water daily). Recheck in 4–6 weeks.
- 52–54%: Action zone. Increase hydration, evaluate TRT dose for reduction, check for sleep apnea and other contributors. Recheck in 4 weeks.
- Above 54%: Intervention required. Reduce TRT dose, consider temporary hold. Therapeutic phlebotomy (blood donation or medical phlebotomy) to reduce hematocrit. Investigate underlying contributors aggressively.
Blood Donation as Management
Therapeutic phlebotomy — removing blood to reduce red cell mass — is the most direct intervention for elevated hematocrit. Many men on TRT manage this through regular blood donation at organizations like the Red Cross. The practical consideration: donation centers may defer donors with very high hematocrit or hemoglobin, and frequent donation can deplete iron stores. Some clinicians prescribe therapeutic phlebotomy through medical facilities that don't have donation-center restrictions.
Hydration's Outsized Impact
Mild dehydration can raise hematocrit by 1–3 points. For a man sitting at 52%, proper hydration might bring the value back to 50%. This isn't trivial — it can be the difference between the watch zone and the action zone. On GLP-1 medications that suppress appetite and cause nausea, proactive hydration is even more critical.

Hematocrit elevation is the most clinically significant safety concern on TRT — and the one that produces no symptoms until complications occur. Monitor it at every blood draw, manage it proactively through hydration and dose adjustment, and intervene with phlebotomy when thresholds are crossed.
